DOH: HERDS Critical Asset Survey - shutdown coming on 12/19/18

This message is to inform all Health Care Facilities (HCFs) - hospitals, nursing homes (NHs) and adult care facilities (ACFs) - that the Critical Asset Survey in HERDS will be temporarily shut down - starting tomorrow (Wednesday December 19, 2018) and continuing through January 1, 2019. 

During this shutdown, the three current forms of the Critical Asset Survey (Baseline/Surge/Resilience) will be merged into one, combined form going forward. All data that was previously submitted in both the Surge and Resilience forms will be migrated to the new, combined form and therefore will not need to be re-entered. 

The Critical Asset Survey is being reactivated on January 1, 2019 - in advance of the rollout of the Facility Evacuation Planning Application (FEPA) in New York City. A Dear Administrator Letter (DAL) regarding the Facility Evacuation Planning Application (previously known as the Facility Profile Application) will be forthcoming to HCFs in NYC. 

The Critical Asset Survey review and update period will run from January 1 - March 31, 2019. All Health Care Facilities are required to review their previously submitted data, update any that has changed and (re)Submit the Critical Asset Survey data during this time period.
