DOH: Office of Health Emergency Preparedness on Facility Survey of Barcode Use

The New York State Department of Health (NYSDOH) Office of Health Emergency Preparedness (OHEP) is researching the feasibility of expanding the eFINDS application to be able to accept facility-generated barcodes. This would allow facilities to enter data into eFINDS and track patient/resident movement through the facility's or network's existing wristband/barcode systems. Your answers to the questions on this survey will help us make eFINDS a seamless and unobtrusive part of emergency evacuations. NYSDOH OHEP continues to work to improve eFINDS so that patients and residents at NYS healthcare facilities can be fully accounted for during emergency evacuations

Please complete this survey to help identify and understand the different methods for patient/resident identification currently in use at facilities. Please ask those staff, who are assigned as eFINDS Reporting Administrators at your facility, to work together to complete this survey. As this is a highly technical survey, you might wish to assure that technical, or information technology staff are also included in preparing your survey response.
